Payment plan possible?

Hi Dr. Cresswell,
Thank you for the quick response. I appreciate it. As for the suggestions, how do I go about scheduling a formal hearing hearing to determine if a payment plan is possible? Is an email to you or Ms. Spooner adequate for this request? 
Likewise, that is great to know that I do not need to go to a drug and alcohol rehab facility especially since there is no reason for a mental/physical exam, but if I did want to submit myself to the LSBME’s egregious demands, how can I go about doing that? Again, I am destitute and living in the state of Florida. What doctor would you recommend I see nearby for this unnecessary mental/physical exam? 
I have completed a second degree program in midwifery which included a clinical component. Is this good enough to satisfy the competency requirement?
I have completed over 300 hours in continuing education. I have sent these courses to Ms. Spooner, but have not received a response. I can forward the list to you for approval.
You stated that after completing all of these requirements, my license MAY be reinstated under probation. Can you please tell me how someone who has not had any poor outcomes ever in ANY state could have their license suspended indefinitely regardless of where I plan to live?
